The forms you will use most often
are listed in the table below, along with a link to access the form
on-line. Following the table are instructions on how to complete the
forms.
On all forms,
please PRINT legibly. Also, please use only non-black pen, so as
to distinguish originals from copies. On multi-part forms, please use
non-black ballpoint pen and press hard.
US Youth Soccer Membership Form (1601)
This is a two-part form. If printing off
the website, please turn in two copies. This form is also known as
the medical release form and must be completed in order to register any
player with CYSA. Instructions for completing the form are below:

appear on your goldenrod. Team names must be unique. If there is
another team in De Anza with the same name as your team, then you must
add the team year to the name, e.g. De Anza FC 90, or some other
differentiator. Force team names should be written as De Anza Force
98G Blue.

CYSA ID# — Fill in the ID#
for all players who were registered in De Anza for the previous seasonal
year. Their ID#s will start with 0218. If the ID# does not start with
0218, leave this blank. ID#s can be found on player passes,
goldenrods and player add forms.
Player information —
must be completely filled in.

Last name — must match last
name on birth certificate/passport. Be careful with adopted children and
remarriages! You need to use the exact name as it appears on the birth
certificate to avoid delay in processing paperwork and potential
ineligibility for tournaments. If last name is hyphenated and player only
uses one name, it is okay to just put that one name.

First name — must match name
on birth certificate/passport. Can use either first or middle name as
first name, but cannot use nicknames. If using two
“first” names, must appear in same order as
on birth certificate.

Middle initial — you can
leave this out, if you choose.
Family information —
must be filled in, minus the sizing information
Liability Release and Medical
Release — both must be filled in, signed and
dated in both places by parent or by player if player is 18 or older.
Note that if the player is 18 or older, the player must sign the
form, not the parent. Must have original
(“wet”) signature in both places of the
form in non-black ink. If using single page form printed from the
web, must turn in two copies, both copies with original signatures in
both places.
Foreign Document Translation
Form
If the Proof
of Age submitted by a player does not include an English translation as
an official part of the document, a Foreign Document Translation Form
must be completed and approved by the district registrar. When you submit
this player's registration, the approved FDT must
accompany the player's Proof of Age. FDTs can be
printed off of the CYSA North or CYSA District II websites.
Adult Registration and Team
Management Disclosure Form
Used to
register all adults working with a team (coach, assistant coaches,
trainers, manager, team assistants). All fields with an asterisk must be
filled in or answered. Pink, full page, multi-part form. If printing the
form off of the website, please turn in three
copies.
Team Roster Change
Form
This form
replaces the Player Add Form, the Player/Adult Release Form, the Player
Transfer Form and the Adult Add Form. The top part of the form not in a
“box” must be filled in for all roster
changes. Depending on which roster change you are making, complete the
appropriate “box” in the center of the
form. The bottom part of the form (signatures for receiving league and
district) must be completed for all roster changes. For each specific
roster change, the following items must accompany this
form:
Paperwork should be turned in
(not folded) in full size envelope with team name, age group, contact
name, and contact email written in upper corner.
Paperwork must be submitted to
league registrar no later than Wednesday, 1 ½ weeks before player
expects to be eligible to play.
Player is
eligible to play the 8th day after the form is signed and
dated by the district registrar (e.g. if signed and dated by district
registrar on Friday, player is eligible to play Saturday a week
later).
