Team Registration for 2007-08 Seasonal Year
(THIS SECTIONIS NOT YET UPDATED FOR 2007-08 REGISTRATION)
If your team
has been approved by DYSL to play as part of the De Anza League for the
new seasonal year, please follow these instructions carefully to insure
the quick and accurate processing of your team's CYSA
registration. To help ensure the viability of teams, DYSL requires a
minimum number of players in order to register a team. These requirements
are:

U10 and Younger: minimum of 10 rostered
players, maximum of 13.

U11 — U19: minimum of 13
rostered players, maximum of 18.
Team
registration packets are distributed in April and usually
include:

Note: If your team will be
playing in a non-CYSA league (eg NorCal), the team must submit a Team
Roster and applicable fees.
Submitting
Paperwork

Place all paperwork in a large
envelope per instructions below.

Write the following in the upper
corner of the envelope (or use envelope with label):
- Team name
- Age group
- Playing league
- Date of 1st CYSA tournament
- Registration contact name
- Registration contact email address

I will assume that the date of
your first tournament is when you need your paperwork

Turn into league
registrar.

Assume four weeks for
turnaround of completed team registration packets (2 weeks with league
registrar and 2 weeks with district registrar).
Required Forms and
Paperwork
On all forms,
please PRINT legibly. Also, please ensure that the parents on your team
use only non-black pen, so as to distinguish originals from
copies.

Playing League Team Registration Forms:
Abronzino (D1), Delgado (D3), or other Playing League Team Registration
Forms filled in from the playing league websites. These should be turned
into the De Anza league registrar only if required by the playing
league (historically required by Abronzino and Delgado; historically not
required by Premier, GSSL).

From Each Adult:

Adult Registration and Team Management
Disclosure Form: pink, full size, multipart form. Must be completed for
all adults working with the team -- coach, assistant coach,
trainer, manager. Fields with an * must be completed in order to
register.

Copy of required coaching license.

Team Roster Change Form: Needed for any
coaches or managers beyond the first three. This is in addition to
the form listed in the previous bullet. Team Roster Change Forms are not
included automatically in your team registration packet. Ask the league
registrar if you need these.

Photo: Refer to details in Players
section, below. Write name on the back of picture. Paperclip picture to
Adult Registration and Team Management Disclosure Form.

From Each Player:

US Youth Soccer Membership Forms, in
their own stack in alphabetical order.

Proof of Age and Name: In a
separate, alphabetized pile: legible copies of player birth
certificates, passports, green cards, refugee cards, foreign birth
certificates, or driver's licenses. Any document that
is not written in English must be accompanied by a completed Foreign
Document Translation Form, approved by the District II Registrar. Please
submit all copies on standard 8.5”x11” paper
— do not cut down to the size of the document as small
pieces of paper are more easily lost. Note that all birth certificates
must have a stamp from the county registrar to be acceptable
(check the back of the original bc if this stamp is not visible on the
front).

Photo:

Size for photo
1”w x 1”h photos of players, coaches, managers,
cut to the correct size, with the name written on the back of
each. Attach to the upper right hand corner of each
player's birth certificate using a paperclip, not
staples.

All photos must be a
“face-shot” showing ears, hair, chin

No hats or sunglasses
-
No previously used player-pass pictures
or otherwise laminated pictures.
